Honbu Dojo Application Form

Please spend a little time considering your answers to the below questions. Try if you can to avoid skipping questions, or one and two-word answers. We are definitely interested in your thoughts about training with us. Answering 'no' or 'unsure' to some of the questions below is acceptable, although we may like to discuss your answers with you in order to better understand your situation. Please note that as a general rule at the North Vancouver Honbu Dojo we prefer new applicants observe at least one full class before beginning training. We will contact you within 48 hours of submitting your application. Thank you.

Do you wish to register in Karate classes or Kobudo classes (Okinawan weaponry) or both?

Do you have any martial arts experience? If so, in which style(s) have you trained and for how long?


KARATE classes at the North Vancouver Honbu Dojo are $60/month and KOBUDO classes (Okinawan weaponry) are $40/month (each program payable with post-dated cheques 6 months in advance). You may train in either or both Karate and Kobudo. Monies collected are used to cover our expenses, mainly the rental of our facility. Students may terminate their membership at anytime and have their cheques promptly returned to them upon request. Please note: there are no refunds for expired cheques. KARATE classes take place Saturdays from 4:30 PM till 6:30 PM and KOBUDO classes, also on Saturdays, from 6:45 PM till 7:45 PM. Are you able to be in regular weekly attendance for the full duration of the classes you wish to join?

We are a community-focused organization and enjoy participating in community events, such as the Vancouver Sun Run (walk or run). We also sponsor free self-defense workshops for the general public at various schools, community centres and neighborhood houses throughout the Lower Mainland. Would you be interested in participating in these events whenever possible?

In martial arts we are trained to show the proper respect for those with greater experience in the art than ourselves. The head instructor is referred to as Sensei, which means: "one who has gone before", or simply: "teacher". Customarily, practitioners will bow to one another in a spirit of mutual respect and gratitude, as well as upon entering and leaving the training area. Are you willing to observe these traditions and rules of etiquette?

You will be required to purchase a traditional uniform and white belt within a maximum of two weeks of beginning training from a local martial arts equipment supplier. The cost is about $50 for a basic uniform or $90 for a premium one. You will be asked to keep your uniform freshly laundered and pressed in preparation for each class. If properly maintained, you may expect 1.5 years of wear from the basic uniform and 2.5 years of wear from the premium. Can you meet this requirement?

There is an annual association fee of $40, payable the first day of training, which includes: special clinics, a student manual, a subscription to our school newsletter, participation in our annual non-competitive meet, as well as our annual beach workout. Funds collected are used to cover our school's expenses in offering these valuable services to our members. Can you meet this request?

The practice of martial arts is a contact oriented, physically strenuous activity. Although serious injuries are rare, one must be aware there does exist a risk nevertheless. No member(s) of Shintokukai Karate & Kobudo can be held responsible for any injuries you may experience. Do you accept this risk?
